I really don't understand what you want. If you need a template with a menu section orientated at the right check the template sections. There are several ones offered. Study them and make your own template.

Shouldn`t it work with the plugin asform (http://cmsimplewiki.com/doku.php/plugin ... ers/asform)? Here you can define as many recipients as you want. And you should be able to place it at any page.
